2005-2006 Physician License Renewal
2005 is a renewal year and approximately 22,000 full licenses are scheduled for renewal before December 31, 2005. As requested by our licensees, the Board has implemented several format changes on the renewal application. The most significant change is the layout of the renewal application, which is now easier to read and provides more detailed instructions. Physicians are advised to start the license renewal process immediately to ensure the timely renewal of their medical license and to be attentive to the following requirements on the renewal application:
Due date - the Board must receive the renewal application and the renewal fee of $400.00 four (4) weeks before the expiration date which is the licensee’s birth date. Renewal applications received after the due date must be accompanied by a $25.00 late fee and therefore the renewal fee will be $425.00.
E-mail address and fax numbers – the Board is collecting E-mail addresses and fax numbers and will use this information to communicate information with licensees more efficiently and effectively.
Office based surgery – physicians who perform any surgery in their offices must complete the PCA-O form included in the renewal packet. This information will be forwarded to the PCA (Patient Care Assessment) office to assess office based surgery standards across the state and is not intended for disciplinary purposes.
PLEASE NOTE: Physicians who fail to renew their license before their birth date cannot practice medicine after 11:59 p.m. on their birthday and their license will lapse. Revival of a lapsed license requires completing a lapsed license application, including a check for the lapsed fee of $600.00 and submitting a National Practitioner Data Bank Profile and AMA Profile. It may take up to three (3) weeks for the National Practitioner Data Bank and AMA to process profiles requests.
